POSITION: HURON PERTH ENHANCED PSYCHOGERIATRIC RESOURCE CONSULTANT
Huron and Perth Counties
Full-Time

POSITION SUMMARY:
Working as a member of the Huron Perth Seniors Mental Health and Addiction Response team, in partnership with Alexandra Marine and General Hospital and working with the Huron Perth Geriatric Cooperative the EPRC role will support the work of Behavioural Supports Ontario (BSO) The Behavioral Supports Ontario (BSO) Initiative was created to enhance services for older adults and their caregivers living with or at risk of responsive behaviours associated with complex and challenging mental health and addictions issues, dementia or other associated neurological conditions. The goal of BSO is to create a system-wide transformation to ensure these older adults living with or at risk of responsive behaviours receive evidence based care in a timely manner.

REPORTS TO: Manager Seaforth Community Hospital

DATE REQUIRED: As soon as possible

QUALIFICATIONS:
Education:
- Registered Nurse, Masters Degree in Nursing preferred
- Current registration with the College of Nurses of Ontario
- Current BCLS
- Certified in Gentle Persuasive Approach (GPA)
- Certified Trainer in PIECES modality
- Training in Comprehensive Geriatric Assessment Plus
- Certified in Montessori approach to Care
- Nonviolent Crisis Intervention Training
- Certification in Psychiatric and Mental Health Nursing preferred.
- Training in Process Optimization and LEAN methodologies preferred

Experience:
- Experience working with older adults with mental health and addictions issues in a hospital and community based setting
- Advanced skills in completing physical, psychogeriatric screening, assessment and consultation
- Advanced knowledge base in Pharmacological interventions for the elderly and in Geriatric Psychiatry
- Demonstrated skill in the completion of Mental Health and Geriatric Psychiatry mental health rating scales
- Extensive knowledge of community mental health and addiction resources and supports in the Huron and Perth counties and the SWLHIN Region

Skills/Abilities:
- Ability to work independently as well as collaboratively with a wide range of professionals
- Ability to maintain case load assignment while fulfilling duties specific to the EPRC position
- Ability to support and work with members of the HPSMH Team, Virtual BSO team members and embedded BSO team members in LTCH in assessing patient care needs, planning care and evaluating patient outcomes
- Experience with program development and evaluation, competence in completion of statistics for program reporting and the ability to analyse trends to support program development
- Self-directed worker, who is flexible and adaptable to meet competing demands
- Ability to respond to patient care needs in a timely manner. The ability to respond to all referrals within two hour standard.
- Strong presentation skills, with an understanding of Adult Education Principles
- Excellent problem solving skills and communication skills
- Computer literacy with proficiency in Microsoft Office Suite
- Access to vehicle, auto endorsement insurance to support patient transport
- Travel throughout SWLHIN
