POSITION: HURON PERTH ASSERTIVE COMMUNITY TREATMENT TEAM
Professional Staff
(Registered Nurse, Social Worker or Occupational Therapist)
Full-Time
POSITION SUMMARY: The Huron Perth Assertive Community Treatment Team will endeavour to reflect the community it serves by providing assertive community treatment and outreach to residents of Huron and Perth counties with long term serious mental concerns.
REPORTS TO: Manager, Outpatient Mental Health
DATE REQUIRED: As Soon As Possible
QUALIFICATIONS:
Registered Nurse Designation or Degree in a Health-Related Discipline; Current Registration
and member of good standing with the College of Nurses of Ontario, or Current registration with
Appropriate Professional College or Association
Current Basic CPR Certification
Completion of a Crisis Intervention Training Program or Equivalent.
REQUIREMENTS:
Minimum five Years Clinical Mental Health Experience
Valid Ontario Driver’s Licence and Access to Own Transportation with Auto Endorsement Insurance for Transporting Clients
SKILLS/ABILITIES:
Functions cooperatively with Huron and Perth multidisciplinary teams
Demonstrates well-developed assessment, counselling, case management, crisis intervention,
planning and service coordination skills
Demonstrates excellent interpersonal, verbal and written communication skills
Self-directed, motivated team player with a commitment to ongoing professional development
Computer literate
