POSITION: TECH 5 - Transitional Case Worker
Huron Perth Helpline and Crisis Response Team
Primary Site: Clinton
Full-Time

POSITION SUMMARY:
The Tech 5 - Transitional Case Worker position is a 24/7 “Safe Bed” program in the Huron Perth district. In partnership with police services (both municipal and OPP), the Transitional Case Worker will assist the Huron Perth Helpline and Crisis Response Team staff with the intake, assessment, and support for individuals entering the “Safe Bed” home. The position will assist with day-to-day needs, resource information, connection and referral for individual clients experiencing crisis situations. The Transitional Case Worker provides crisis intervention and a safe therapeutic environment for individuals to work through their crisis situations. The Transitional Case Worker provides the support and advocacy to assist these individuals to navigate the community support system while in the “Safe Bed” home. Staff will be expected to work from the Clinton “Safe Bed” home in this position and participate in the practical running of the home.

REPORTS TO: Manager, Outpatient Mental Health

DATE REQUIRED: As soon as possible

SALARY: $32.18 to 37.05 per hour

QUALIFICATIONS:
Education:
Required
- Registered Social Service Worker or Addiction Counsellor
- Current registration with a regulated college in Ontario such as OCSWSSW
- Current licensure with appropriate college or association
- Extensive knowledge of the Recovery Principles and Recovery Model
- Completion of Non Violent Crisis Intervention Training
Preferred
- Completion of a crisis intervention certificate program (preferred)
- Certification in Brief Solutions Focused Therapy (preferred)
- Completion of suicide training program (preferred)
- Non-Violent Crisis Intervention Training (preferred)
- Current CPR and First Aid certification

Experience:
- Minimum 3 years current progressive clinical experience with individuals with serious mental illness and /or addictions in a health care setting and/or community-based setting
- Experience in responding to crisis situations with a variety of individuals in the community and/or hospital setting

Abilities:
- Demonstrated knowledge of issues faced by individuals in acute mental health/addiction distress and by individuals with severe mental health illness
- Knowledge of community services and mental health, addiction treatment and support programs
- Demonstrated knowledge and competence in the application of treatment modalities to support individuals with serious mental illness and/or addiction
- Knowledge of mental health reform and values, principles of recovery, Mental Health Act, PHIPPA, Health care consent Act and Public Hospitals Act
- Demonstrated ability to conduct formal mental health and addiction assessment, short term crisis counseling and referral
- Personal transportation and hold a valid Ontario driver’s license
- Auto endorsement insurance for the purposes of patient transportation
- Demonstrated ability to complete treatment plans and crisis plans with the client, community partners and colleagues
- Self-directed, motivated team player with a commitment to ongoing professional development and client and family centred care
