POSITION: Central Intake Worker - Psychiatric Day & Evening Program and Listowel Mental Health Outpatient Services
(Registered Nurse or Social Worker)
Regular part-time

POSITION SUMMARY: The successful candidate will work with individuals primarily 16 and above. The role includes working with outpatients, completing comprehensive assessments to determine needs and facilitate referrals to appropriate mental health, addiction and/or community resources. This role also includes identifying individuals requiring urgent psychiatric consultation and competency in expediting service delivery. Experience in working with individuals with serious mental illness and/or addiction issues is essential. The applicant must be organized and self-driven, highly skilled in collaborating with community agencies, have an interest in family intervention, a superior knowledge of psychotropic medications, and be competent in crisis intervention. This program will service Huron and Perth Counties which includes seeing individuals at satellite locations as well as being able to respond to individuals requiring in home assessment.

REPORTS TO: Program Manager, Outpatient Mental Health Services

DATE REQUIRED: As soon as possible

QUALIFICATIONS:
- Bachelor degree in social work from an approved school of social work
- Current registered member in good standing with the College of Nurses of Ontario or the Ontario College of Social Workers
- 5 Years Mental Health experience preferred with preference given to individuals who have worked in the community
- Preference will be given to who have a demonstrated understanding of addictions and the recovery model.
- Valid Ontario Driver’s License and Access to Own Transportation with Auto Endorsement Insurance for Transporting Clients

SKILLS/ABILITIES:
- Knowledge of treatment modalities and group facilitation is an asset
- Demonstrated knowledge with psychopharmacology
- Demonstrated adult teaching skills
- Effective communication and interpersonal skills
- Excellent assessment skills & development of treatment plans
- Crisis intervention skills
- Computer literate
- Experience working with Psychiatrists, Family Physicians & other professionals
- Willingness to travel throughout Huron & Perth counties and the SWLHIN
- Willingness to work flexible hours to ensure that patient care and program needs are met
