ABOUT THE ROLE

The primary responsibilities of this position will be to conduct decision guide intakes for clients referred to the Chronic Pain Management Program.

The Social Worker provides services to clients and/or their Substitute Decision Maker (SDM) family members and other support persons in the client's life when applicable. From herein the job description will refer to clients and/or others for this purpose. The Social Worker is responsible for providing comprehensive assessment and intervention that addresses the psychosocial needs of the clients and/or others. The Social Worker is an integral part of the interprofessional team providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice. Identifies opportunities for and participates in quality improvement activity within the care setting.

The goals of Social Work practice are to restore maintain and enhance the quality of life by mobilizing strengths supporting coping capacities assessing and supporting people to modify patterns of relating and acting linking people to necessary resources addressing environmental stressors individually/ collectively and providing psychosocial education related to wellness and subjective well-being.

WHAT YOU BRING

Education/Experience:

  • Baccalaureate degree in Social Work (BSW or HBSW)
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Thorough knowledge of the Code of Ethics and Standards of Practice as set out by the Ontario College of Social Workers and Social Service Workers
  • Knowledge of social work values and ethics theory and established practices of clinical social work
  • Understanding of the social determinants of health of indigenous people and the social consequences and risk factors associated with client conditions (ie.addictions and mental health chronic pain)
  • Training and experience related to client care for people from Indigenous geriatric addictions and mental health chronic pain populations
  • Knowledge of family systems and family dynamic skills in working with families
  • Knowledge of community systems and resources for the purposes of networking between the facility/program and other services
  • Indigenous educational background directly related to social work preferred

 

Skills/Abilities:

  • Ability to manage a clinical practice and caseload in a health care setting
  • Ability to complete analysis using clinical knowledge skill and judgement to synthesize assessment information
  • Assessment and treatment of psychosocial issues
  • Ability to communicate effectively and concisely both orally and in writing
  • Ability to work independently interdependently and participate as a contributing member of a variety of teams within the workplace community and ministries/agencies
  • Strong time management analytical/critical thinking problem solving and decision making skills
  • Ability to use independent judgment and maintain confidentiality of records actions and information
  • Demonstrated regular attendance at work

 

Asset:

  • Indigenous language fluency and/or Indigenous cultural education is an asset
  • Clinical experience and/or training in chronic pain is an asset

 

Conditions of Employment:

  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
  • May be required to attend various work locations and provide own transportation.
